Long-Term Services & Supports (LTSS)

Supporting independence, fostering individual choice, and improving quality of life through coordinated, person‑centered programs

As long-term care needs evolve, healthcare organizations must design programs that improve health outcomes and quality of life for members while managing cost efficiency for states and plans. Led by registered nurse case managers, our team brings clinical, community-based, and direct care experience to support high-quality, appropriate services.

Our Work in Action

Aided in 3,900 facility-to-community transitions

Since 2003, we have helped a Medicaid agency support 10,000+ medically complex people by completing 23,500+ eligibility assessments and processing 5.2 million claims, enabling 3,900+ transitions.

$161 million

Savings from medically necessary LTSS decisions

For a Medicaid agency, we managed more than 140,000 LTSS prior authorizations annually, ensuring services were medically necessary, non duplicative, and regulation compliant—producing over $161 million in cost avoidance since 2003.
