We operate at the intersection of justice and public health, helping to close service gaps, improve continuity of care, reduce recidivism risk, and ensure health transitions that support long-term reentry success.

Our Work in Action

Improving care transitions and cross-system coordination

We partnered with state Medicaid, corrections, and community providers to launch a behavioral health initiative for over 2,400 justice‑involved individuals, improving care navigation, reducing overdoses, and strengthening cross-system coordination.

Enhancing care navigation for successful reentry

We partnered with the Massachusetts Parole Board to expand clinical care navigation, improving connections to treatment, housing, and supports—contributing to a 15% reduction in revocations and a 76% successful parole discharge rate.
