Streamlining Disability Determinations for a State Medicaid Agency

The Challenge

Our client, a state-level health and human services agency, was experiencing significant delays and operational challenges within its Medicaid disability determination process. Extended timelines, inconsistent workflows, and a backlog of more than 11,000 pending cases created persistent delays and heavy operational strain on internal staff.

In addition to these core operational challenges, the agency depended on an external partner to gather medical records before determinations could proceed. Although a process existed to streamline this step, rising volumes made it difficult for the partner to keep pace. This resulted in delays, adding yet another bottleneck to an already strained system.

Referred by a client we previously supported with similar challenges, the agency engaged ForHealth Consulting to streamline and enhance its disability determination operations.

The Solution

We began with in-depth discovery sessions and assessments, then partnered with the agency to transition the majority of its disability determination process from a manual, paper-based system to an electronic workflow using its existing infrastructure. This transformation delivered stronger privacy protections, real-time data sharing, and seamless document transfer among applicants, clinicians, and determination staff. To ensure accurate processing and clean record separation, we implemented unique applicant identifiers in the electronic system. While most of the disability determination process has become electronic, a portion remains with the state agency, which continues to operate on paper. However, our streamlined approach and use of unique identifiers enabled smoother coordination with the paper-based process.

To help address the growing backlog, we stepped in to support the medical records vendor by creating a Pre Review Process. While this work was outside our original scope, it was uniquely suited to our team’s expertise. As the volume of record collection increased, our specialty clinicians—vocational experts trained to interpret longitudinal medical information and make accurate disability determinations—were able to review existing records and identify cases that did not require additional medical record collection.

We also demystified the disability determination process by meeting with key stakeholders in the Medicaid community, including a pediatric oncology practice and a large hospital system. Additionally, we agreed to participate in pre-conference hearings for applicants found ineligible for Medicaid benefits due to disability. These sessions, held prior to formal hearing proceedings, foster open dialogue among applicants, agency personnel, and ForHealth Consulting, strengthening understanding of the disability determination process and reducing the number of formal hearings required.

The Result

By streamlining workflows and implementing a paperless system, ForHealth Consulting not only met the client’s 45-day processing target but exceeded it—completing disability determinations in an average of just 11 days per case. Specifically, key results include:

Reduced average processing time from 45 days to 11 days
Improved coordination between the electronic workflow and the remaining paper‑based processes—creating smoother handoffs and more reliable case movement
Completed 1,950 disability determinations between August 2024 and October 2025, with only ten appeals and zero overturned decisions—demonstrating strong first‑time accuracy
Supported the medical records collection vendor in advancing 394 cases through the Pre Review Process by identifying cases that did not require additional medical record collection—accelerating decisions and reducing administrative burden
Reduced the number of formal hearings through stakeholder engagement and pre‑conference participation—resolving many cases earlier in the process and decreasing administrative burden
Improved transparency and applicant experience—fostering trust

These outcomes did not just resolve immediate challenges—they set a new benchmark for efficiency, accuracy, and compliance in Medicaid disability determinations. By transforming a fragmented, paper-heavy process into a streamlined, secure, and scalable model, ForHealth Consulting empowered the agency to deliver faster decisions, improve transparency for applicants, and position itself as a leader in Medicaid modernization.
