Connecticut Environmental Scan on Community Health Workers

Our ForHealth Consulting team recently provided the Connecticut Health Foundation with a scan of Medicaid payment actions for community health worker services, looking at the 50 states.

The report highlights state practices when it comes to paying for community health workers’ services, including fee-for-service and managed care models, and compares services eligible for payment.
