What is the problem/waste? No log in sheet in Pathology for specimens going to tissue bank for paras. Idea: Pathology will put sign in sheet up for patient name and date and the name of person delivering specimen. Date the idea was implemented: 7/1/2017 |

NAME: Donna White Area: Memorial Clerical-Scheduling What is the problem/waste? Inadequate information on the Lung Screening requisition causing a need for additional communication Idea: Information that was needed from providers was not on the current Lung Screening requisition. A new lung screening form was created and an additional form to communicate any further needs before a patient could be scheduled. This proved to be a more comprehensive questionnaire for the referring physicians and more efficient for the scheduler. Date the idea was implemented 7/5/2017 |

Inefficient Tech work-flow, lack of accountability for staff to work as a team Idea: The work-flow for the techs was not efficient. They changed roles from scanning to patient and room prep after each patient. A new work flow that assigned a tech to scanning, pat/room prep and procedures for the entire morning and then changed in the afternoon allowed for a more efficient work-flow, increased productivity and accountability. They come to ER radiology front desk to ask us to contact ER. Idea: There is a sign over the phone in the family rooms with a number to call for any information on loved ones. Date the idea was implemented: 7/13/2017 |
