UMass Chan Medical School is expanding its COVID-19 vaccine requirement to include a booster shot. We are asking all UMass Chan faculty, staff and students who have already received a COVID-19 booster shot to upload a photo of the vaccination card and the date the booster was administered to Service Now at your earliest convenience, following the instructions below. If you have already received your COVID-19 booster, the deadline to upload this documentation is Jan. 10, 2022.
If you are not yet eligible to receive a COVID-19 booster (eligibility begins six months after the second dose of the Pfizer or Moderna mRNA vaccine or two months after the J&J vaccine), Service Now will generate a reminder email when you do become eligible for a booster. Upon receiving this reminder, UMass Chan faculty, staff and students will have three weeks in which to get a booster shot and upload documentation via Service Now.
Vaccination, testing and other public health measures are the most impactful tools we have to protect the health and safety of our colleagues, our campus environment, and our broader community. We urge you to follow best practices over the holidays and every day. Leadership will continue to closely monitor data related to cases and to the spread of the highly transmissible Omicron variant; we anticipate sharing additional guidance after the first of the year.
If you have already received a COVID-19 booster, please complete the following steps prior to the Jan. 10 deadline:
- Log into the UMass Chan Service Now portal using the mobile app or web browser and follow the steps to upload the date and the image of your booster vaccination record.
- If you are unable to access Service Now, you may email an image of the front of your vaccine card to Theresa.kane@umassmed.edu.
- You may also bring your vaccination card to the COVID-19 testing site in the Faculty Conference Room on the main campus and staff will upload a photo for you.

COVID-19 booster vaccines are widely available, and appointments are recommended. You can find a location and schedule a shot at https://vaxfinder.mass.gov/.
The U.S. Food and Drug Administration has authorized COVID-19 boosters for everyone age 16 and up, and the U.S. Centers for Disease Control and Prevention’s website has helpful guidance and answers to common questions.
