Geriatric Medicine is an interdisciplinary approach to the comprehensive care of older adults. Our geriatricians and geriatric nurse practitioners focus on the complete picture of patients to promote wellness, enhance function and preserve independence. These goals require the evaluation of medical as well as psychosocial issues, review of medications, goals of care, social supports and available community resources along with cognitive and mobility assessments.
Common issues encountered in older patients include multiple chronic illnesses, urinary incontinence, falls, memory issues, polypharmacy, caregiver stress and side effects from multiple medications. Services available in the geriatric medicine clinic include primary care of the older adult, comprehensive geriatric assessment, fall assessment or consultation for medication review or cognitive assessment.
Staff members, including geriatricians and geriatric nurse practitioners, perform home visits for our most frail patients and most many members of the division provide care in several of the area’s skilled nursing and long-term care facilities. Geriatric practitioners work as part of an interdisciplinary team of physical and occupational therapists, social workers and case managers to provide comprehensive care.
