This patient's speech is fluent and melodic, but it has little or no meaningful content. The patient understands little of what is said to her, and cannot follow simple commands whether they are spoken or given in writing. She does not move away from most potentially threatening visual stimuli when they are presented on the right side and involve the upper parts of her visual field. She does pull back from similar stimuli that are presented on her left side. She shows no weakness on strength testing, and has normal reflexes including the plantar response. Touch sensation is normal on both sides. Cranial nerves 3-12 are apparently normal, although testing is difficult because of the patient's problems with language. This patient has had an ischemic stroke.
