• Patients should see their primary care provider for a referral.

 UMass Memorial Health Care Clinicians:

• UMass Memorial Health clinicians may refer a patient for outpatient TMS via EPIC.
• To refer a patient for ECT, please call 508-334-1055.

 Non-UMass Memorial Health Care Clinicians:

• Non-UMass Memorial Health clinicians should utilize the Referral Form.
