UMass Chan Community Health Toolkit
Including a population perspective when considering health problems can be key to understanding what's happening in your clinic. |
Our department is happy to be working with the Coalition for a Healthy Greater Worcester, who serve as the lead convener for implementation and evaluation of the Worcester Community Health Improvement Plan. See below for the top twelve policy change campaigns for the next five years, and check out the Coalition website to learn more and get involved.
You can still see the impact of the nine priority areas identified in the 2016 CHIP process, which guided funding and activity in the region for the years 2016-2021.

Priority Area 2: Substance Use
Priority Area 3: Access to Care
Priority Area 4: Mental Health
Priority Area 5: Economic Opportunity
Priority Area 6: Cultural Responsiveness
Priority Area 7: Access to Healthy Food
Priority Area 8: Physical Activity
Priority Area 9: Safety
This toolkit was originally created in 2010 to:
> Provide access to high-quality population health data and measurement tools
> Link providers to population-based and community resources
This is YOUR TOOLKIT - and a work in progress. Foundational materials were developed and uploaded in 2010, and while we've made some updates, there is definitely room for improvement. If you have ideas to improve this site or would like to join the team working to make it more useful, please contact Heather-Lyn Haley, Program Director for Community Health.
