STUDENT MEDICAL RECORD

All new students are assigned a Medical Record Number within the UMASS Memorial Heathcare System.  Student health information is entered into the UMASS electronic health record (Epic).  Records are held to HIPAA standards to maintain confidentially.  After graduation, students may request their health information from the Medical Records Department at (508) 334-5700 option 1.
