Award Nomination Forms

UMass Chan Special Award - Directions to submit the Nomination Form and the Recommendation Letter are included within the Form.

UMass Chan Manager Nominated Spot Awards - Directions to nominate an employee for a "Lunch On Us Award", or an "Appreciation Award" are included within the Form. Upon approval of recipient recieving award, the recipient may select their gift by completing the Manager Nominated Gift Selection Form on the Recognition website. Directions to select a gift are included within the Form.

Employee Thank You Card - Directions to request hardcopy thank you cards are included within the Form.

Chancellor’s Award for Advancing the UMass Chan Mission - is bestowed annually upon an individual UMass Chan staff member or team whose actions make a substantial impact in advancing the mission of the University of Massachusetts Chan Medical School through: public service, research, teaching, and healthcare delivery
