Enroll in Short-Term Disability

To enroll in the Short-Term Disability Plan, you must complete the Guardian STD Enrollment Form.

To access the STD Enrollment form, click the Benefit Forms button below (or on the top navigation bar) and the STD Enrollment Form will be located within the Short-Term Disability Forms section.
