Why Barre Family Health Center: After practicing and gaining experience I wished to expand to both learn and provide more comprehensive care to patients of all ages. Corey Costanzo, DO, MPH, MS

Assistant Professor

Boston Medical Center, 2022
Rocky Vista University College of Osteopathic Medicine, 2019
Clinical/Academic Areas of Interest: Women’s health and obstetrics, family planning, rural medicine, osteopathic medicine, public health.
About UMass: After finishing residency in Boston, I was looking for a clinic where I could provide quality, full spectrum care, including maternity care and osteopathic treatment, for families. Barre Family Health Center has a long history of dedication to the surrounding communities while also having the benefit of being a part of the UMass system allowing patients access to continuity and specialty care. I also very much enjoy clinical teaching and sharing the knowledge I have learned with the next generation of physicians. I feel very grateful for the opportunity to become a part of the Barre team and work with such a caring group of staff and providers.
Personal Facts: My training has taken me from coast to coast, from Boston to Colorado back to Boston and finally home to central Mass. I could not be more excited to return and provide care to the communities I grew up close to! Outside of work, I spend most time with family and friends. I enjoy the outdoors and am always trying to fit in as much skiing, paddleboarding, and hiking as possible!

Alex Doering, MD

UMass Family Medicine Residency, Barre
Geisel School of Medicine at Dartmouth College
Bowdoin College
Clinical/Academic Areas of Interest: Full-spectrum family medicine, geriatrics, palliative care
About UMass: UMass Chans Medical School was founded with the goal of teaching providers how to provide primary care to the Commonwealth, and UMass Memorial Medical Center and the Worcester Family Medicine Residency align with that goal.  The residency program is also unique, in that we have three very different outpatient clinic sites.  Unlike a lot of other residency programs, where you have to match and then enter a lottery for a specific clinic site, our program allows residency applicants to rank each clinic site in the Match.  Despite the different outpatient clinic sites, residents overlap on all inpatient rotations, and it is fun to get to learn from residents at other clinic sites
Why Barre Family Health Center: I am from a rural town in Maine, and I was initially drawn to Barre Family Health Center for residency because it allowed me to practice outpatient medicine in a rural setting, while also getting to practice inpatient medicine in the second biggest city in New England, Worcester. Barre is also a very procedure-heavy clinic, and I love the breadth of procedures we get to see - everything from laceration repairs to endometrial biopsies.  I am excited that I get to stay as an attending physician and see all of my former co-residents graduate from the residency program
Personal Facts: I love cooking, hiking, and movies. My partner and I are currently gearing up for a northbound through-hike of the Appalachian Trail in 2024

Stephen Earls, MD
Associate Professor of Family Medicine
Former Education Director and Medical Director

Columbian College of Arts and Sciences of The George Washington University
The George Washington University, MD
Miriam Hospital Internal Medicine Residency
University of Massachusetts/Worcester City Hospital
Clinical/Academic Areas of Interest: Rural Health, Healthcare and Practice Management
About UMass: Our health centers are the strength of our program. Located in the community rather than in the hospital, our residents enjoy an opportunity to be a genuine community family physician while experiencing the excellent training offered in a major university medical center.
Why I Chose Family Medicine: I grew up with a general practitioner providing care to my family and was impressed with the way in which he was able to make it all seem like it would turn out all right. Meeting a community need as he did became my ambition and I felt I could only do that in Family Medicine. While the science of medicine is interesting, the relationship with patients is what I enjoy.
Personal Facts: Avid but not very good golfer. Proud father and grandfather.

  Kristina Gracey, MD, MPH

About UMass: It is an honor to be working at UMass, practicing full-fledged Family Medicine at the Barre Family Health Center while delivering babies and taking care of patients of all ages in the clinic and at UMass Memorial Hospital. I have a particular enthusiasm for teaching medical students and residents and feel fortunate to be surrounded by colleagues who share similar passions and have unique clinical skillsets.
Personal Facts: My husband and I, alongside our three young children, love hiking mountains locally and in different parts of the world, meeting new people in interesting places and taking care of our many farm animals. The beautiful rural environment full of centenarians, strong academic community, diverse Worcester community, and countless opportunities for personal and professional growth brought me to (and keep me at) UMass.

UMass Chan Family Medicine Residency, 2019
Clinical/Academic Areas of Interest: Full-spectrum Family Medicine, Inpatient Medicine, Rural Health education
About UMass:  As a UMass “lifer” (I did my undergrad, medical school, and residency all within the UMass system), I have come to appreciate all that UMass has to offer. The residency program is the perfect blend of high-quality training with amazing support for the residents. There is so much opportunity for learning and growth; when the time came to look for a full-time position, I realized that UMass was the best place to pursue my goals of being an academic full-spectrum family doctor in Massachusetts.
Why Barre Family Health Center: I love our patient population, the people I work with, and the opportunities we have to be a part of the community and practice full spectrum care.
Personal Facts: I enjoy cooking, reading, and keeping bees and chickens.

Cynthia D. Jeremiah, MD
Medical Director
Assistant Professor

Medical University of Silesia in Katowice Poland, 2005
Residency at UMass Chan Family Medicine; Barre Family Health Center, 2009
Clinical/Academic Areas of Interest: Rural Healthcare, Adolescent Health, Women’s Health
About UMass: BFHC in my mind defines family medicine as you take care of individuals from the beginning of life until the end.  You take care of their parents, partners, children, and many of the other people who intersect their lives.  In addition to preventative health care and chronic disease management, we also provide procedural care, urgent care, and maternal-fetal care.  We care for our patients when they're admitted to UMass Memorial Hospital on our own in-patient service.  And we care for them when they're admitted to Holden Nursing Home and Rehab for either long-term care or short-term rehab.  You can't get more complete than that. We have learners at all levels, including Medical and APP Students, and FM Residents.  It’s a wonderful place to work and learn. It can get pretty busy, but it never loses its small-town charm.
Personal Facts: I'm a New Yorker, and will be a Yankees and Giants fan forever.  My husband and I enjoy travelling, visiting historical sites and museums, cooking, going concerts and shows, and playing UNO.  I enjoy reading nd music, and spending time with my family and friends.

About UMass: I went to UMass Amherst for undergrad, then UMass Boston for a post-baccalaureate program, then started work at UMass Memorial as an ED Tech before going to PA school. After a quick year working in Connecticut, I came back to UMass Memorial Health at Barre Family Health Center in 2014.
Why Barre Family Health Center: My first practice out of PA school was with a Barre-trained physician, so when the time came to look for another job, I was thrilled to have the opportunity to interview in Barre. The full-spectrum, whole-person approach to care in Barre is the type of practice that makes it easy to come to work every day, especially when surrounded by faculty and staff who are dedicated to their patients and community. I appreciate that Barre is a place of continual learning, teaching, improvement, and innovation.

Sarah Pearson, PsyD (she/her)
Assistant Professor

Doctorate of Clinical Psychology, Antioch University New England
Predoctoral Internship, University of Colorado School of Medicine
Postdoctoral Fellowship, UMass Chan Medical School, Department of Family Medicine and Community Health
Clinical/Academic Areas of Interest: My clinical interests include providing brief, evidence-based interventions in integrated primary care, delivering patient-centered, trauma-informed care, chronic pain management, behavioral sleep medicine, health behavior change, and chronic disease management. I am passionate about teaching behavioral science curriculum and biopsychosocial topics to family medicine residents, and continuing to explore and develop ways in which we can best support residents’ learning and wellbeing based on their unique needs and experiences throughout their medical training.
Why Barre Family Health Center: My current role as a faculty member is actually the third position I have held at Barre! My Barre journey started when I was a psychology practicum student receiving my first introduction to practicing primary care psychology, and I learned so much about providing behavioral health interventions in integrated primary care while working alongside incredible family medicine providers. I knew that my first choice for my postdoctoral fellowship was going to be UMass, and I was so grateful to be able to return to Barre to expand upon my teaching and clinical skills as a fellow. And now, I can say that returning as a faculty member in our clinic truly feels like coming home!
Personal Facts: Outside of work, I spend my time with my partner and our goldendoodle, visiting with family and friends, practicing mindfulness, riding my horse, and traveling as much as possible.
