Student Health Clearance Form

Respirator Fitness Form - This medical clearance form is to be completed by all first year T.H. Chan School of Medicine and Tan Chingfen Graduate School of Nursing students (Morningside Graduate School of Biomedical Sciences student are excluded). An e-mail will be sent by Student Health to direct you when the form is to be completed. Once medically cleared by Student Health, Environmental Health and Safety will complete the mask fitting process. Please download this form and return to SHS. TST Form - This form must be completed in lieu of an annual Quantiferon Gold  If you have had a positive TST in the past please answer the first 2 questions and symptom review section and return to Student Health each year. **NOTE: Morningside Graduate School of Biomedical Sciences students are not required to have annual TST testing. Tan Chingfen Graduate School of Nursing students are required to have a Quantiferon Gold in clinical years only TB Screening Form
